FEES:
- If an applicant is 16 or older the passport fee is $75.00.
- If an applicant is 15 or under the passport fee is $60.00.
- Additional processing fee of $25.00 is due to the Clerk of Court either by cash or check on both of the above passport applications.
PASSPORT EXECUTION GUIDELINES:
- If the applicant is 18 or older, the applicant must sign the application and present his or her own identification.
- If the applicant is 14 to 17, the applicant must sign the application and must submit evidence of at least one parent’s consent or awareness. If the applicant lacks his or her own identification, the parents’ may be used.
- If the applicant is under 14, the applicant must submit proof of relationship to parents (typically by submitting a birth certificate or adoption decree that lists parent(s) name(s), even if the child has a previous passport. The applicant also needs both parents’ signatures on the DS-11, or one signature and a completed notarized DS-3503 (a custody order or death certificate may be required).
Regardless of age, all applicants must be present when applying.
WHAT YOU NEED TO HAVE WITH YOU WHEN APPLYING FOR A PASSPORT:
- A certified copy of your Birth Certificate.
- Two (2) identical photographs of yourself, size 2 inches by 2 inches (2 x 2).
- Check or money order made payable to U.S. Department of State for applicable fee (separate check for each applicant).
- Cash or check made payable to the Jackson Parish Clerk of Court for processing fee ($30 each applicant).
NOTE:
- Ordinary time for getting passports is 4-6 weeks. If needed sooner, you can choose the expedited service. If this is chosen, you will need to purchase one overnight envelope and bring in when you apply for your passport. A total fee of $151.25 takes care of this overnight return. ($136.25 for age 15 or under)
